The route a patient uses for treatment, the government fee as the source states it (“Free” where the source publishes no charge), how many people can travel on companion visas, and whether the application is online. Where a fee is “not published”, the reason follows.

Why no government fee is shown: Dubai's official page gives two different fees for the same visa, AED 200 and AED 300 for 90 days and one entry, so we cannot show one. It also says the total may vary.
